Please contact our office if you do not see your insurance plan listed on this page. There are many times that we can still see you while we complete the paperwork necessary to participate with your insurance company.  

Remember, there are numerous product lines owned/operated by the payers.  It’s always important to consult your directory to ensure you can see us.  

Referrals & Authorizations

Many payers, especially HMOs, require that you obtain an authorization before being treated by a specialist.  Please consult with your primary care provider and request a referral to our office if your plan type requires it.  If you’re not sure, you should consult with your insurance handbook.  You can also check the back of your insurance card as it’s often indicated here.

 

No Show/Cancelation Policy

Need to cancel or reschedule an appointment?  No Problem!  Just make sure to give us 24 hours notice.  No Show Fee $75.  Same Day Cancellation Fee $75.
